BackgroundWe previously described a procedure for eliciting deep spatial discrimination of individual segments in the healthy lumbar spine of normal subjects: the percutaneous mechanical provocation (PMP) test. Our goal was to devise a method for accurate identification of the spinal level of pathology in chronic low back pain (CLBP). In the present study, we validated the PMP test, using a subgroup of CLBP patients with isthmic spondylolisthesis (IS). Because there is clinical consensus that IS back pain originates in the slipped segment/disc, the level of pathology can be directly compared to the result of the PMP test. The test is agnostic with respect to the underlying pathological mechanism, and therefore might be useful in identifying the involved segment(s) irrespective of the painful structure.MethodsIn 37 patients with confirmed IS (slippage 3–15 mm), we compared sensitivity between the PMP test, the widely used provocative discography test and the discoblock test.ResultsThe PMP test reliably identified the slip level in patients with IS, with sensitivity of 92%. Accepting the slipped disc as the origin of pain in IS, the sensitivity of the provocative discography and discoblock tests were 49% and 35%, respectively: i.e., too low to be contributive in clinical practice.ConclusionsThe PMP test reliably identified the origin of localized pain in IS as the slip level, but should be used with care in CLBP patients in selecting discogenic pain patients for fusion surgery, since the specificity of the test is not known and it may be positive for any origin of localized pain.  相似文献

BackgroundIndividuals with knee osteoarthritis (OA) show various dynamic sagittal-plane changes during the early stance phase of gait. However, the effect of these kinematic alterations on knee load during the early stance remains poorly understood. Research question: The purpose of this study was to examine the effect of altered sagittal- plane knee kinematics on knee load during the early stance.MethodsA total of 13 healthy adult men underwent gait analysis trials using four conditions (baseline and three altered conditions). The three altered conditions were defined as follows:1) Less flexion (LF): a gait that decreased knee flexion excursion (KFE) owing to a reduced peak knee flexion angle compared to baseline.2) Initial flexion (IF): a gait with decreased KFE owing to an increased knee flexion angle at initial contact, during which the peak knee flexion angle did not differ from baseline.3) Flexion gait (FG): a gait that increased the knee flexion angle at initial contact but did not reduce KFE compared with the baseline.Data analyzed included peak external knee flexion moment (KFM), KFM impulse (impulse was an integral value from initial contact to peak value), peak vertical ground reaction force (VGRF), and maximum loading rate.ResultsBoth LF and IF conditions significantly decreased peak VGRF (p < 0.05) compared with the baseline. Peak KFM decreased in the LF condition and increased in the FG condition versus baseline (p < 0.05). A significantly increased KFM impulse was found in both IF and FG conditions when compared with baseline (p < 0.05).SignificanceAn increase in knee flexion angle during early stance increased knee loading. Interventions are likely required for improving excessive knee flexion during early stance phase of gait in individuals with knee OA.  相似文献

Background

Despite numerous benefits, only a small fraction of laparoscopic left-sided colectomy is accomplished without the need for an abdominal incision to retrieve the specimen and prepare for anastomosis. We report our early experience with a robotic approach using Natural orifice IntraCorporeal anastomosis with Extraction of specimen (NICE) to help overcome the technical limitations and challenges of this approach.

Methods

Twenty consecutive patients presented for elective sigmoid or rectosigmoid resection for benign and malignant disease and underwent the NICE procedure. Safety, feasibility and post-operative outcomes were analyzed.

Results

Intracorporeal anastomosis was accomplished in all patients. One patient required an abdominal incision to extract a bulky tumor. Mean operative time was 222?min (146–344). Mean time to first flatus and length of stay was 23 and 49?h, respectively. All but 4 patients were discharged home on post-operative day 2. One patient was readmitted with a pelvic fluid collection.

Conclusion

Robotic left-sided colorectal resection with NICE procedure is a safe and feasible minimally invasive approach and may facilitate greater adoption rates of this technique.  相似文献
